The standard Medicare Part B premium is $202.90 a month in 2026, an increase of $17.90 from $185.00 in 2025.
That is the figure most people pay. Roughly 8% of enrollees pay more because of an income-related surcharge based on their tax return from two years earlier, and the annual Part B deductible is a separate $283.

How It WorksHow the Part B premium works
Part B covers doctor visits, outpatient care, lab work, durable medical equipment and preventive services. Nearly everyone pays a premium for it, unlike Part A, which is free for anyone with 40 quarters of Medicare-covered work. If you receive Social Security, the premium is deducted from your monthly benefit automatically; if you do not, Medicare bills you quarterly.
You still pay the Part B premium when you join a Medicare Advantage plan. The plan does not replace it.
Who Pays MoreWho pays more than $202.90
Higher earners pay an income-related monthly adjustment amount, or IRMAA, on top of the standard premium. For 2026 the surcharge starts above $109,000 of modified adjusted gross income for a single filer and $218,000 for a couple filing jointly, measured on the 2024 tax return. At the top bracket the total Part B premium reaches $689.90 a month.
Signing up late also costs more. The Part B late enrollment penalty adds 10% of the standard premium for every full 12-month period you could have had Part B and did not, and it is permanent.
Next StepsWhat to do next
Check which bracket your 2024 return puts you in before assuming you pay the standard rate. If your income has since dropped because of retirement, a job loss, divorce or the death of a spouse, file form SSA-44 with Social Security and ask them to use your current income instead.
If you are turning 65 or leaving employer coverage, enrol during the window that applies to you so no penalty attaches.
More QuestionsPeople also ask
Is the Part B deductible separate from the premium?
Yes. You pay $202.90 a month and, separately, the first $283 of Part B services each year before Medicare starts paying its 80% share.
Does everyone pay the same Part B premium?
No. About 92% pay the standard $202.90. The rest pay an income-related surcharge, and some pay a permanent late enrollment penalty on top.
Has CMS announced the 2027 Part B premium?
Not at the time of writing. CMS normally publishes it in November. Any 2027 figure circulating now is a projection, not the rate.
