A hospital stay on Medicare costs $1,736 in 2026 as the Part A inpatient deductible, charged per benefit period rather than per year.
That covers the first 60 days. From day 61 you pay $434 a day, and from day 91 you draw on 60 lifetime reserve days at $868 a day. Because it is per benefit period, two separate admissions in one year can mean paying the deductible twice.
| Days of the stay | What you pay |
|---|---|
| Days 1 – 60 | $1,736 deductible, then nothing |
| Days 61 – 90 | $434 a day |
| Days 91+ (60 lifetime reserve days) | $868 a day |
| Skilled nursing, days 21 – 100 | $217 a day |

Benefit PeriodsWhat a benefit period actually means
A benefit period starts the day you are admitted and ends once you have been out of hospital and out of skilled nursing care for 60 days in a row. Only then does the clock reset. This is the detail people miss: the $1,736 is not an annual deductible, so a January admission and a September admission are two separate charges.
Where It Gets LargeWhere the exposure gets large
Original Medicare has no out-of-pocket maximum. A long stay or a run of admissions has no ceiling, which is the single biggest difference between Original Medicare and a Medicare Advantage plan, where an in-network maximum caps the year. Skilled nursing adds its own $217 a day from day 21, and an observation stay that was never formally an admission may not count toward the three-day rule that unlocks that benefit at all.
Next StepsWhat to do next
If you are on Original Medicare, this exposure is what a Medigap policy or a hospital indemnity plan is bought to cover — the first pays the cost-sharing, the second pays you cash you can use for anything. If you are on Medicare Advantage, find your plan’s in-network maximum out-of-pocket, because that is your real ceiling for the year.
Ask whether you are admitted or under observation every time. The answer changes what Medicare pays.
More QuestionsPeople also ask
Is the $1,736 deductible charged once a year?
No. It is charged per benefit period, so more than one admission far enough apart means paying it more than once in a calendar year.
Does Medicare cap what I can pay in a year?
Original Medicare has no out-of-pocket maximum. Medicare Advantage plans must have an in-network maximum, which varies by plan and county.
What are lifetime reserve days?
Sixty extra days you can use across your whole life once a single stay passes 90 days, at $868 a day in 2026. Once spent, they are gone.
